We are a not-for-profit, clinically-led organisation that supports 12 GP practices and 3 PCNs within the Durham Dales across Wear Valley, Bishop Auckland and Teesdale.
As a NHS provider, DDHF services are delivered by 100 dedicated healthcare professionals, working in close partnership with primary and secondary care, local authorities and the voluntary sector.
It’s encouraging to see the NHS 10 year plan driving neighbourhood working and a shift of care from hospitals to community based services.
DDHF already facilitates the Integrated Neighbourhood Team partnership in the Durham Dales and is well placed to continue delivering services in the community.
Our Services
DDHF employs multi-disciplinary teams of skilled and experienced clinicians practitioners, who support patients and service users with their health and wellbeing, and wider economic, social and lifestyle issues. These services are reducing our GP practices workload and helping to avoid unnecessary impact on the wider system.
Private Services
DDHF now offers a limited number of private services to help us grow sustainably and diversify our income.
These Minor Surgery and Physiotherapy services are delivered by our highly skilled workforce and offer patients choice in a clinically safe setting across County Durham. DDHF also delivers Basic Life Support Training either at your place of work or at Innovation House, Bishop Auckland.
We continuously strives to improve patient experience and ensure a consistent high quality level of care.
In March 2025 Care Quality Commission (CQC) rated us “Good” across all domains – Safety, Effectiveness, Caring, Responsiveness and Leadership.
